What are the benefits of advance care planning? What is an advance healthcare directive? An advance care directive is an important part of your end-of-life care. The directive can contain all your needs, values and preferences for your future care and details of a substitute decision-maker.


This document allows an adult to document their preferences for future medical treatment, should they lose decision-making capacity. A person can record general statements about their values and preferences to guide.

It is a written statement of your wishes to refuse a certain treatment. It may also include the specific situation in which you wish to refuse the treatment. It is a way of making sure that everyone knows what treatment you do not want to have, if you become unable to make your own decisions in the future. The Values History is a two-part advance directive instrument that elicits patient values about terminal medical care and therapy-specific directives. The goal of this advance directive is to move away from a focus on specific treatments and medical procedures to a focus on patient values and personal goals.
